Community & Recreational Use of School Facilities Form

This form must be completed and returned to the middle school office at least 10 days before any event.

Attached is the WMS Add-Drop Form. This form is used by students and parents to add or drop an unrequired elective course. Those with questions about adding or dropping a course are welcome to contact Principal Bob Caelwaerts at 532-5553 or by email at caelwaerts@wrightstown.k12.wi.us.

Report of Bullying/Harrassment Incident

Parents and students are welcome to use the attached form to report bullying or harassment that occurs during the school day. Please return completed forms to Mr. Caelwaerts and Mrs. Ederer. The staff of WMS is committed to resolving incidents of bullying and, or harassment in a timely manner.
